LMRA — Last Minute Risk Analysis
A Kiwa or VCA auditor asks whether your installers carry out an LMRA before every job. Your paper forms are somewhere in a van, and nobody knows whether the de-energising check was actually done.

What it does
- Step-by-step flow with large touch buttons — full screen per question, so an installer can complete it wearing gloves.
- Question types: YES/NO, YES/NO/N.A., text, photo-required, checkbox list.
- Critical questions — on NO the app blocks the work automatically.
- Approval flow: the installer can request a justification from a supervisor, with a mandatory rationale + additional measures.
- GPS coordinates recorded so the audit trail matches the work-order location.
- Work-order link: a work order cannot be AFGETEKEND (signed off) without a valid LMRA dated on or after the work-order date, if the installation is marked 'LMRA required'.
- Append-only — no edit, no delete. A 7-year retention period (VCA requirement).

How installers use this module day to day
Maintenance visit on a flat roof (working-at-height risk)
The installer arrives at a flat-roof installation and opens the LMRA on their phone. The question 'Am I working at height? If so, is fall protection in place?' is critical. Answer NO → the work is blocked. The installer calls the supervisor, gets additional measures and requests approval with a rationale. Only then can the work order start — all with an audit trail.
Emergency repair outside office hours
An installer alone at a leak in the evening. The LMRA forces a check on refrigerant-detector calibration + ventilation + lone-working risk. On a CRITICAL NO the supervisor gets an email and has to approve explicitly — no unsafe work slipping under the radar.
Leak check at a retail supermarket
A workplace with bystanders. The question 'Are there bystanders at risk?' is informational; on YES the installer gets the hint 'Cordon off the area'. No block, but auditable evidence that the check was done before work started.

Frequently asked questions
Is an LMRA mandatory?
Under VCA-VOL a documented LMRA per job is mandatory. Under VCA-Basis it is strongly recommended. ISO 45001 principles (occupational health and safety management systems) call for an equivalent risk-analysis flow.
What if an installer skips the LMRA?
You can tick 'LMRA required' per installation. The work order then cannot be AFGETEKEND (signed off) without a valid LMRA. The installer sees a blocking message with a direct link to the LMRA flow.
Does it work offline?
The question flow works locally. Answers are held in IndexedDB and sync as soon as there's a network again. GPS and photo upload need a short burst of connectivity at submit time.
Can I add my own questions?
Not yet. Every tenant currently uses the same fixed eight-question base template for refrigeration work (the first six critical) — a template builder per installation type is on the roadmap.
How do supervisor approvals work?
On a CRITICAL NO, every OWNER and ADMIN in the workspace gets an email. Anyone who wants it can also turn on a push notification on their phone (Settings → Notifications), so the alert still lands if the email isn't seen right away. The supervisor has to explicitly justify why the work goes ahead anyway and what additional measures were taken; that approval is tied to the LMRA record and can't be edited away.
What do auditors see?
At /lmra/uitvoeringen you export a PDF/CSV with every LMRA in a period: installer, date, installation, outcome (VEILIG / ONVEILIG_GESTOPT / ONVEILIG_DOORGEZET — safe / unsafe-stopped / unsafe-continued), GPS, answers per question and any approvals. Meets VCA audit requirements.
